2024 - 2025  Athletics Registration

 Winter Registration Deadline  - November 4, 2024   

 

Step 1:  PHYSICAL FORMS  - The NJ Dept of Education has changed the sports physical process 

  • Please follow the next steps carefully.and print the forms from the link provided below. 
  • The link takes you to a list of health forms. The only health forms that need to be printed and taken to the physicial appointment are as follows: 

Health History Forms (2 pages)   

Annual Athletic Pre-Participation Examination Form 

  PPE Medical Eligibility Form 

 

  • PPE MEDICAL ELIGIBILITY FORM IS REQUIRED TO BE SUBMITTED TO THE NOTRE DAME NURSE'S OFFICE.  Email to NURSE@ndnj.org

 

  • Print the NJ Dept of Education required physical forms HERE

 

  • Make sure the PPE Medical Eligibility Form is stamped by the doctor's office.  
  •  Both NJ and PA residents must submit the PPE Medical Eligibility Form.
  • Physicals are valid for 365 days and must be current in order to participate in athletics.
  • The state of New Jersey requires that the doctor performing the physical has completed The Cardiac Assessment Professional Development Module.  

 

  • IMPORTANT - HEALTH HISTORY UPDATE QUESTIONNAIRE - If the student-athlete's physical is over 90 days, the Health History Update Questionnaire must be completed by a parent and submitted to the Nurse's Office in order for the athlete to tryout. This form can be scanned and emailed to Nurse@ndnj.org. You can print the HHU Questionnaire HERE

 

    THE DEADLINE TO SUBMIT HEALTH FORMS FOR 2024 FALL SPORTS IS 

November 4, 2024 

 IF PHYSICAL IS SCHEDULED AFTER THAT DATE IN NOVEMBER, PLEASE NOTIFY OUR OFFICE at Tattory@ndnj.org

 

Step 2: SPORTS REGISTRATION  MODULE -  REGISTER BY NOVEMBER 4, 2024  

 

Click HERE to Log in to your Family Account (Returning User) or Create a New Account . 

  • For returning users, the fields will auto-populate with the information from the previous registration

 

  • For first time users, please leave Student ID field blank. It is an optional field. If you have an incoming freshman and they participated in a camp at ND, the student is still considered a first time user for this system.

 

If you have issues with athletic registration, athletic username and passwords please DO NOT set up another account under a different name. Contact Chris Tattory in the Athletic Office at (609) 882-7900 ext 105.
