Fall Sports Registration Starts June 16, 2025

FALL TRYOUT DATES
August 11, 2025 - Football and Girls Tennis
August 18, 2025 - Boys Soccer, Girls Soccer, Field Hockey, Cross Country, Cheer, and Volleyball
To register fully for fall sports, the following two steps must be completed. Since the state has migrated to a new system this year, a new account must be set up for all students.
Step 1: REGISTRATION MODULE
- Click HERE to register on the new Arbiter athletics registration module. All students will require a new account.

STEP 2: HEALTH FORMS
- Print the Preparticipation Physical Evaluation Medical Eligibility Form HERE
- The PPE MEDICAL ELIGIBILITY FORM must be uploaded to the athlete's enrollment checklist in School Admin.
- Notre Dame requires the doctor's office to stamp the PPE Medical Eligibility Form.
- Both NJ and PA residents must submit the PPE Medical Eligibility Form
- The state of New Jersey requires that the doctor performing the physical has completed the Cardiac Assessment Professional Development Module. Forms without the doctor's signature on that section of the form cannot be accepted
- Physicals are valid for 365 days and must be current to participate in athletics
- IMPORTANT - HEALTH HISTORY UPDATE QUESTIONNAIRE (HHU): This form is only necessary if the physical was completed more than 90 days before the first day of tryouts (any physical dated before May 18, 2025, or May 11 for Football and Girls Tennis) The HHU must also be completed by a parent and submitted to the Nurse's Office for the athlete to try out. THE DEADLINE TO SUBMIT HEALTH FORMS FOR FALL SPORTS IS
AUGUST 1, 2025
Please notify our athletic office if your student's physical is scheduled after August 1, 2025.
